Correlation between the Observer and the Self-Rating Scales in Patients with Major Depressive Disorder

Abstract
Background and Objectives£ºWe examined the relation between the observer ratings and the self-reported ratings about depression or anxiety symptoms in patients with major depressive disorder.

Materials and Method£ºOne hundred twelve patients with major depressive disorder from outpatient clinic were recruited. We administered the observer depression and anxiety rating scales, the self reported depression and anxiety rating scales£ºHamilton Rating Scale for Depression (HAM-D), Hamilton Anxiety Scale (HAM-A), Zung Self Rating Depression Scale (Zung-D), and Zung Self Rating Anxiety Scale (Zung-A). We compared HAM-D scores with Zung-D scores and HAM-A scores with Zung-A scores in all subjects. We also compared the HAM-D scores with Zung-D scores, and HAM-A scores with Zung-A scores in the patients by severity of depression. We calculated Pearson¡¯s correlation coefficients between the depression rating scores and the anxiety rating scores. We used the¡°Statistica¡±program for statistical analyses.

Results£ºThe total scores of HAM-D showed strong correlation with those of the Zung-D in depressed patients (r=0.63, p<0.0001). HAM-D scores showed stronger correlation with ZungD scores in patients with severe depression than in patients with moderate or mild depression (severe£ºr=0.60, p<0.0001, moderate£ºr=0.34, p<0.05, mild£ºr=0.56, p<0.01). The total score of the HAM-A showed strong correlation with those of the Zung-A (r=0.74, p<0.0001). HAM-A scores showed stronger correlation with ZungA scores in patients with more severe depression (severe£ºr=0.84, p<0.0001, moderate£ºr=0.64, p<0.0001, mild£ºr=0.63, p<0.01).

Conclusion£ºCorrelation between the observer rating and the self-rating anxiety or depressive symptom scales was stronger in patients with more severe depression. It is necessary to measure the severity of depression with discretion as to apply both the observer and self-reported depression ratings together in patients with less severe depression.
